Suggestion: In Final Forms, select the checkbox for every sport your student is even slightly interested in. This will ensure that you/your student receives information about practices and tryouts, and will allow you to sign all of the required waivers and forms now, rather than having to scramble at the start of each season. Please Note: Users will be required to sign forms once per year and anytime an update is made. 

ASB Fees (Paid During the School Year)

Students participating in Athletics are required to pay a participation fee (ASB). These funds directly support the cost of athletic programs: game transportation, equipment, uniforms, and more.

Athletes must provide proof (receipt) for final athletic clearance. To securely purchase ASB for the school year you will use your SchoolPay account starting in August each year.

  • Pay ASB fee and/or any additional team fees or fines on SchoolPay via the Source or in person at the Fiscal Office or on the direct SchoolPay link as well.
    • Student Athletes participating (and Band) are required to pay a participation ASB fee. These funds directly support the cost of athletic programs: game transportation, equipment, uniforms.

Physicals

Up-to-date Physicals must be good from and be current from the first day of tryouts/practices until the day after the State Playoffs. Refer to the WIAA Sports Season Calendar tab for dates. Late paperwork may limit participation for tryouts and practices.

Physical Expiration Dates are able to be viewed and are located on students Source Accounts and though FinalForms within a week of being turned in to the Athletics Secretary (during the school year). Physicals are valid for 24 months from the Exam Date  listed by the doctor on the Physical Form. Please make sure your doctor includes the exam date, and a readable doctor’s stamp with signature. Visit the district Athletic Forms page for more information.

Note: Middle school physicals do not automatically transfer to High School, in order to ensure student-athlete info is current and correct physicals for incoming freshman will need to be resubmitted to the BHS Athletics Secretary.

Once the physical is completed , there are four ways that you can turn in the physical to the Athletics Secretary, all four work great.

  • Upload a scan or a photo of the physical directly through final forms. Log in and then select the “Student’s Documents” folder button in the actions section of your student’s listing
  • E-mail a picture or scanned copy of the physical directly to the BHS Athletics Secretary, Bethany Nielcen at blnielcen@seattleschools.org
  • Drop a paper copy of the form directly in the main office
  • Fax the form to the main office at 206-252-1001 – Your doctor’s office will sometimes agree to do this for you

Return to Play Form – Required if you are seeking a physician to give medical clearance to return to athletic participation after a sickness or injury. It must be returned to Loka Murphy, Athletic Trainer loka.murphy@seattlechildrens.org or the Athletics Bethany Nielcen; blnielcen@seattleschools.org
