Please fill out and return the required Start of School forms by September 30, 2026. The forms are available to download w/translations on this page. Paper copies of forms are available to pick up outside the BHS Main Office.

All of the required and informational forms for the 2026–27 school year are available on the Seattle Public Schools website or on the district Start of School section below. For those who prefer, paper copies are available for pick up in the Ballard HS Main Office.

- Online: Emergency Info and Student Release Form & Health Update: The Student Data Verification Form can be completed Online via the Source until Sept. 30, 2026.
- Confirm we have your most up-to-date information. This Online Form includes Emergency Contacts, Release Info, and Student Health updates.
- Available to complete online on the Source until September 30, 2026.
- View or print district Emergency Info and Student Release Form.

- Apply for Free/Reduced Meals – In addition students may be eligible for reduced fees; AP exams, ASB fees. Complete online or if paper form, return if applicable to the Ballard HS Lunchroom.
- FERPA – FERPA, Sign & return. If not returned automatic Opt-In by September 30, 2026.
- Family Notifications: Protection of Pupil Rights (PPRA) Survey: Learn more on the district site
- Laptop Device Agreement Form. Laptop Device Agreement – Please complete and return.
- Form 504-2 (Identify Students with Disabilities) – Return 504-2 if applicable.
- Housing Questionnaire (McKinney-Vento Program) – Return Housing Questionnaire if applicable.
- 506 Indian Student Eligibly Form (New Students Only) – Return 506 form if applicable.
- Washington Guaranteed Admissions Program Opt-Out Form – Complete if applicable.

- Students, Parents/Guardians can drop off in the Main Office or,

Back to School 2026-27
Seattle Public Schools (SPS) welcomes our students, staff, and families back to school!
Opportunity begins here! The start of the school year is a time of fresh beginnings and renewed energy. From our youngest scholars to the senior class, our goal is to help all students explore their possibilities in learning spaces where they feel safe, seen, and heard.
School leaders, teachers, and staff are busily preparing to welcome students back to school on Wednesday, Sept. 2.
Get the 2026-27 school year off to a great start by being prepared with the information you need!
- 1st-12th grade students Wednesday, Sept. 2.
- Preschool and kindergarten students Tuesday, Sept. 8
The first day of school is not an early release Wednesday. Early release will occur every Wednesday except for the first week of school.
