Helpful Tips & FAQ for Registering on FinalForms

Ballard HS is using the online system, FinalForms, for families to complete athletics registration for our student athletes. In addition, athletes can register for multiple seasons at the same time! Using FinalForms helps us ensure safety and compliance and allows families to complete their Registration/Paperwork and sign most forms from a computer, tablet, or mobile device and it also translates forms into multiple languages for our EL families.

FinalForms pre-populates information when possible, saving time if you have multiple student athletes and saves data from season-to-season, so no need to enter the same info twice. Additionally, users may login any time to update information including emergency medical contacts and health information, so that coaches and trainers have the most up to date information.

Please Note: Users will be required to sign forms once per year and anytime an update is made.

*Final Athletic Clearance from the BHS Athletic Office is required prior to participating in any WIAA sports.

Start FinalForms Registration Instructions

Guardians must start the FinalForms registration process before student athletes can log into their account and set their password. Both student and guardian are required to complete and sign all forms before students can tryout or start practicing.

You will need the following information for your student athlete to allow for accurate completion of the Online FinalForm Registration Process
⃞   |  Students Personal Email Address
⃞   |  Basic Medical History & Health Information
⃞   |  Insurance Company & Policy Number
⃞   |  Doctor & Dentist Contact Information
⃞   |  Hospital Preference
⃞   |  A Completed Physical
⃞   |  Any extra WIAA required forms (Running Start, New to SPS, Out of District, etc.)

For Unified Sports you will still fill out FinalForms, but final clearance comes from Robyn Maddess. See the Ballard HS Athletic Directory for coaches contact information.

Physicals

Up-to-date Physicals must be good from and be current from the first day of tryouts/practices until the day after the State Playoffs. Refer to the WIAA Sports Season Calendar tab for dates. Late paperwork may limit participation for tryouts and practices.

Physical Expiration Dates are able to be viewed and are located on students Source Accounts and though FinalForms within a week of being turned in to the Athletics Secretary (during the school year). Physicals are valid for 24 months from the Exam Date  listed by the doctor on the Physical Form. Please make sure your doctor includes the exam date, and a readable doctor’s stamp with signature. Visit the district Athletic Forms page for more information.

Note: Middle school physicals do not automatically transfer to High School, in order to ensure student-athlete info is current and correct physicals for incoming freshman will need to be resubmitted to the BHS Athletics Secretary.

Once the physical is completed , there are four ways that you can turn in the physical to the Athletics Secretary, all four work great.

  • Upload a scan or a photo of the physical directly through final forms. Log in and then select the “Student’s Documents” folder button in the actions section of your student’s listing
