Note! Checkout forms will be distributed on Monday, June 8 or Seniors can pick up a form in the Main Office or Counseling Center. Graduation Tickets will be picked up at the Checkout process.

Senior Checkout Form & Details

Congratulations! You need to complete this entire form to be eligible for commencement activities. All school items need to be returned and fines cleared in order to participate in the graduation ceremony.

Your Counselor will be the last signature you get on Thursday, June 11

You may get the following signatures starting Monday, June 8 before/after school or at lunch.

• Laptop and Charger Clearance
  • See Room N110 (check hours) or Library
• Library/ALL Textbook Clearance
  • See TuesD Chambers or Britta Bowman, Library
• Debt/Fine Clearance
  • See Latt Thant, Fiscal Clerk in the Main Office (SE130)
  • OR
  • See Say Tuitele, Counseling Center

Clean out your locker, leave lock with the door open. Report any damage or lost lock to Young Wong, Custodian.

Important

Seniors need to attend all classes, complete finals, return laptops, chargers, textbooks, clear fines, and get signatures from their teachers each class period. Please do not ask teachers to sign before June 8 (Finals.)

**Teachers – Do not clear students until they have returned your books and materials to you.
***IEP Teacher signature (required for students who have an IEP) The IEP teacher signature verifies you have met graduation requirements according to the current IEP.

Ticket Distribution

ALL tickets will ONLY be distributed on June 11, 2026 as part of the Senior Check Out Process. It is the VERY last thing after seniors have successfully completed their check out with their counselor on Thursday, June 11, 2026. The tickets are in the Main Office.

Seniors will receive a SEALED individually labeled envelope that has THEIR graduate ticket (labeled + brick red and white checkered) and their assigned guest tickets (labeled + brick red).
