    Following Governor Inslee’s emergency proclamation on March 12, 2020, all Seattle Public Schools will be closed through at least April 24.

    All school building activities including school day instruction, child care, preschool, athletics/sports, health services, enrichment, etc. are canceled until further notice.

    Athletic Paperwork/Packets

    Full Athletic Packets can be picked up outside the Main Office and turned into to the Office as well or download the paperwork below and submit your packet electronically by sending a PDF version of your completed packet to our new athletics secretary, Rayna Bauer, at RCBauer@SeattleSchools.org with "Sports packet" in the subject line.

    What to Do…

    1. Pick up or Download sports packet.
    2. Schedule and Complete your physical if needed. Your physical expiration date shows on your Source page. Schedule your physical if need and remember the BHS Teen Health Center does free physicals for students too! A Physical is valid for 24 months from the date of the exam and must be valid through the day after the state tournament the student is participating in.
    3. Turn In Sports Packets/Paperwork to Athletics Secretary in Main Office
    4. Show up to tryouts/first day of practice

    Did You Know...

    You can submit your paperwork packet electronically by sending PDF versions of your completed packet to our new athletics secretary, Rayna Bauer, at RCBauer@SeattleSchools.org with "Sports packet" in the subject line. Athletic paperwork can also be turned in directly to Rayna at the main office.

    Annual $35 ASB Activity Fee MUST be paid before turning in athletic paperwork. Please bring your Student ID with a current ASB sticker with you when turning in paperwork. You can pay online via SchoolPay on the Source menu.

    Sports Physicals are required prior to tryouts and the first practice. Physicals are valid for 24 months from the “exam date” on the Physical Exam Form. Your doctor must specify in writing: Full or Limited Participation describing limits. Include the doctor’s readable stamp with signature.

    BHS Teen Health Center (BTHC) - Free Sports Physicals! Students can stop by the Teen Health Center to make an appointment for FREE sports physical or call 206-781-6400.

    Insurance is a must. Current Health Insurance is required for all athletes. Enter Insurance Info & remember to sign the form.

    Grades, 2.0 GPA in 5/6 Classes. All students must have and maintain a 2.0 GPA or greater, in 5/6 classes to participate. See the SPS Student Athletic Handbook for more details.

    Refund Policy. The ASB Activity Fee is an annual fee and is non-refundable.

    Are you ELIGIBLE to participate? Where you live, your grades, attendance, and enrollment status could affect your eligibility to participate. If you have transferred from another school into Ballard HS this year, attend a Private School student; or live outside the Ballard HS attendance area please see Rayna Bauer Athletic Secretary in the main office or the Athletic Director Eric Ensign in the Activity Center for additional instructions.
