HPV Vaccine Clinic

In partnership with the American Cancer Society, Swedish is hosting a FREE outdoor vaccination event for HPV, COVID-19 and Flu vaccinations at Ballard High School.

All vaccines are administered at no-cost. Seattle Public School students, faculty, families and community members are welcome.

No appointments or registration necessary, however if you would like to submit your information prior to the event, scan the QR code to fill out form.

The parking lot will feature a gallery walk of educational resources and a raffle to win a $100 Visa gift card.

When: Saturday, Oct 9, 11:00 a.m. – 1:00 p.m.

Where: Ballard High School – Parking Lot 1418 NW 65th St, Seattle, WA 98117

(Parking lot located behind the school off NW 67th and Alonzo Ave NW)

Eligibility: HPV: 9 -19 years old COVID-19: 12 & older Flu: 6 months & older

Cost: None! Free! Vaccines will be provided and administered for free.

Registration is not required, however if you would like to fill out your information prior to the event, please fill out this attached form.

*Minors will need parental consent for the flu vaccine. View or download the Pediatric Permission to Vaccinate Form. Forms will also be available at the event.
