Spring Packets Due NOW thru Feb 10, 2017 | Get Your Packets In Before Mid-Winter Break | Don't Delay!

if you participated in a Fall or Winter sport this year!
YOU need to do 4 things if you participated in a Fall or Winter sport this year:
1. Turn in a Sport Safety Release Form. Baseball/Softball
or Boys Soccer
or Track & Field
or Coed Tennis
2. Confirm with Ms. Nickerson
in main office in person or by email
that your Sports Physical is current through May 28, 2017.
3. Provide a copy of Health Insurance Card if your insurance changed.
4. Turn in the new: Weight & Fitness Room Inherent Risk Consent Form

Did You Know...
Are you ELIGIBLE to participate? Where you live, your grades, attendance, and enrollment status could affect your eligibility to participate? If you have transferred from another school into Ballard HS this year, attend a Private School student; or live outside the BallardHS attendance area please see the Athletic Secretary in the main office or the Athletic Director in the Activity Center for additional instructions.
Annual $35 ASB Activity Fee required: ASB Activity Fee is $35. Make your Check to: BHS or pay by exact cash amount. Sorry, no Credit Cards.
Sports Physicals are required prior to the first practice. Physicals are valid for 24 months from the “exam date”. Physical Exam Forms are included in this packet. Your doctor must specify in writing: Full or Limited Participation describing limits. Include the doctor’s readable stamp with signature.
BHS Teen Health Center (BTHC) - Free Sports Physicals! Students can stop by the Teen Health Center to make an appointment for FREE sports physical or call 206-781-6400. See more info here or on BTHC on our Website under the Services Tab. REQUIRED for services: Parent/Guardian Consent Form.
Insurance is a must: Current Health Insurance is required for all athletes. Enter Insurance Info & sign on page 2 of the registration form.
Grades, 2.0 GPA in 5/6 Classes: All students must have and maintain a 2.0 GPA or greater, in 5/6 classes to participate. See the SPS Student AthleticHandbook for more details.
Refund Policy: The ASB Activity Fee is an annual fee and is non-refundable.

- Return to Play Form - Required if you are seeking a physician to give medical clearance to return to athletic participation. It must be returned to Loka Murphy, Athletic Trainer.
